To enable us to process a reward claim, please complete this form with the following information:

• Card Recovery Reason
• Visa or MasterCard
• Card Number
• Expiry Date
• Cardholder Name
• Date Card Recovered
• Merchant Number
• Merchant Name
• Merchant Address

Cut the card in half horizontally, just under the magnetic stripe, and attach to the form where indicated. Once complete, please mail to the address stated on the form, affixing sufficient postage to ensure delivery.
